The Non-negotiables from a Support Coordination Team.

Our Support Coordinators are here to support those with disabilities or complex needs to access and navigate various support services. In order to Inspire you, these are our non-negotiables so be sure you are getting these no matter what provider you are with:

  1. Person-Centered Approach:
    • Focus on your goals, preferences, and needs.
    • Involve you in decision-making and planning.
    • Recognise and respect your autonomy and choices.
  2. Communication:
    • Maintain clear and open communication with you, your family, and relevant service providers.
    • Ensure information is shared appropriately and in a timely manner.
    • Use accessible and inclusive communication methods.
  3. Understanding of Individual Needs:
    • Conduct a thorough assessment of your needs and circumstances.
    • Keep abreast of changes in your situation and adjust the support accordingly.
  4. Knowledge of Available Services:
    • Stay informed about available support services, pro-grams, and resources.
    • Assist you in accessing a broad range of services to meet your diverse needs.
  5. Collaboration:
    • Work collaboratively with you, your family, and relevant service providers.
    • Foster a collaborative approach among different sup-port services to ensure a holistic and integrated approach to your care.
  6. Advocacy:
    • Advocate for your rights, ensuring you receive appropriate and quality support.
    • Support you in advocating for yourself when possible.
  7. Cultural Competence:
    • Be culturally competent and aware of diversity in needs and preferences.
    • Respect and incorporate cultural considerations into the planning and coordination of support.
  8. Empowerment:
    • Empower you to make informed decisions and build your capacity to self-manage where possible.
    • Provide information and education to enhance your understanding of available options.
  9. Flexibility:
    • Be flexible and adaptable in responding to changes in your circumstances or goals.
    • Adjust support plans as needed to meet evolving needs.
  10. Documentation and Record Keeping:
    • Maintain accurate and up-to-date records of support plans, agreements, and communication.
    • Ensure documentation complies with relevant regulations and standards.
  11. Continuous Improvement:
    • Seek feedback from you and involved parties to continuously improve the coordination of support.
